A fantastic opportunity has arisen for a

Registered Social Worker

to join the

South East Community Wellbeing Team

within

Stoke-on-Trent Adult Social Care and All Age Commissioning

, recently rated

'Good'

by the

Care Quality Commission (CQC)

.

In this role, you will manage a varied caseload, working under key legislation such as the

Care Act 2014

,

Mental Capacity Act 2005

, and other relevant legislation & guidance underpinning adult social care. Our core purpose when working with adults is:

Key Responsibilities:



Conducting Care Act Assessments, reviews, and support planning

Undertaking Safeguarding Enquiries and Mental Capacity Assessments

Maintaining accurate and up-to-date client records

Producing high-quality reports and demonstrating strong communication skills
