We're looking for two passionate and multilingual individuals to join our Primary Care Assessment Team (PCAT). This is a fantastic opportunity to work in the heart of Southall, supporting local residents who may be struggling with alcohol or drug use and are not currently accessing structured support.
You'll be based across Southall GP surgeries, working in partnership with GPs and our wider RISE treatment team to provide early engagement, brief psychosocial interventions, and onward referrals to specialist care.
To meet the needs of our local Southall community, applicants must be fluent in English and one of the following community languages: Punjabi, Gujarati, Hindi, Farsi or Arabic. This is essential for effective communication and culturally sensitive care.

You will be working closely with Southall residents, particularly those from Punjabi, Gujarati, Hindi, Farsi, and Arabic-speaking communities, to provide early support around alcohol and drug use within GP surgeries. As part of our

, you'll help identify and engage individuals who may not yet be accessing structured treatment, offering culturally sensitive and language-appropriate support at the earliest opportunity.

: 27,449.53 - 31,529.41 Dependent on experience (Based on full time hours, Pro Rata for part time hours)
Allowance:
4,072.06 Inner London Weighting, paid Pro Rata

Fixed term for 12 months
Please note:
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
Responsibilities:
Your main responsibilities will include:
Supporting people to create personalised goals to reduce drug or alcohol use
Acting as an advocate and connecting people to services that support their wider needs (housing, benefits, mental health, etc.)
Offering emotional and practical support through active listening and motivational interviewing
Helping individuals build confidence and social skills so they can take part in community life
Delivering care in a culturally sensitive way that meets the needs of Punjabi-speaking patients and their families
About You:
We're looking for someone who is:Confident speaking Punjabi and English
Passionate about making a difference in drug and alcohol services
Experienced in supporting vulnerable people (mental health, substance use, or community settings)
Aware of the challenges people face around substance use and the wider impact on families and communities
A good communicator who can build trust and therapeutic relationships
