Lambeth Single Point of Access (SPA) are a team of mental health professionals, support workers and social workers who can help you access one of three Living Well Centres. These offer short and longer term support from our teams of clinicians, social workers, people with lived experience and support workers, as well as other services and community groups who can support you depending on your need.
We are a part of the Lambeth Alliance, and we have a close interface with a variety of SLaM services, other statutory organisations (inc. social care) and voluntary sector.
The Lambeth Living Well Network (LWN) Alliance brings together Certitude, Lambeth Clinical Commissioning Group, Lambeth Council, South London & Maudsley NHS Foundation Trust and Thames Reach organisations to deliver an innovative way of providing mental health support for those who need it in the Lambeth Borough; a fully integrated and coordinated mental health system which focuses on people's strengths and supports their whole health and wellbeing.
As part of our journey to realising a fully integrated system, we have set up 3 Living Well Centres in Lambeth. The SPA is the front door for mental health services in Lambeth.
We are looking for dynamic and enthusiastic candidates, with strong leadership skills and a commitment to delivering high standards of care.
Please note this vacancy may close early once capacity has been met.
The post holder will work with the Team Lead and Clinical Expert to support and advise on the delivery of high-quality safe care.
You will co-ordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members, and other services within the Alliance to ensure effective interface and continuity of care between disciplines and services.
You will have expertise in leading conversations about people's health and wellbeing and be able, within the context of the triage proforma, carry out mental health and risk assessments and give advice on general health & wellbeing and self-management advice.
Based on the triage discussion, you will give clear direction on what clinical intervention is indicated and whether this is advised within a primary shared care model or within secondary care.
You will support the development of joint working opportunities and protocols alongside other Network organisations for people who do not need a more formal offer, and as a member of staff working within the Alliance you are required to encourage and foster collaborative and cross organisational working and with our wider community services.
So, if you are a skilled mental health nurse who has effective and compassionate communication skills, and who is committed to delivering high quality healthcare advice, then this is a fabulous opportunity for you to join our team. We actively encourage you to contact us to find out more.
We are committed to ensuring our staff get the most out of their benefits package and understand the importance of a healthy work life balance. There is an extensive range of excellent benefits for you and your family. We want you to feel like you are part of a close-knit team at SLaM. It's important to us that you valued and appreciated and that are why we have a comprehensive benefits package on offer
