Senior Specialist CHC Assessor
to lead on complex CHC assessments and contribute to high-quality care planning for individuals with ongoing health needs.
About the Role
In this pivotal role, you will be responsible for leading and coordinating
Decision Support Tool (DST)
assessments--both virtual and face-to-face--to determine eligibility for NHS Continuing Healthcare funding. You will manage a complex caseload and support individuals and families throughout the CHC process with professionalism and empathy.
You will work closely with a range of stakeholders including care providers, local authority partners, and internal teams to ensure care packages are person-centred and appropriate, regardless of CHC eligibility outcomes. The role also includes carrying out
Section 117 reviews
and playing an active role in service development and improvement initiatives.
Key Responsibilities
Lead and coordinate CHC assessments and reviews in line with national guidance
Manage a clinical caseload from assessment through to outcome and care planning
Collaborate with professionals across health and social care to arrange care packages and placements
Provide expert advice on CHC eligibility, Section 117 aftercare, and joint funding pathways
Maintain accurate clinical and administrative records to support data reporting and governance
Contribute to performance targets, service development, and quality assurance
Support training, staff induction, and continuous professional development across the team
Person Specification
Essential:
Registered with the
Nursing and Midwifery Council (NMC)
or
Social Work England (SWE)
Significant post-qualification experience in a health or social care setting
In-depth knowledge of CHC frameworks, the
Mental Capacity Act (MCA)
, and consent processes
Strong communication, assessment, and report writing skills
Confident using digital systems and managing your own caseload
Full UK driving licence and access to a vehicle for community visits
Desirable:
Experience with
Section 117 aftercare
Background in
mental health
,
learning disabilities
, or
older adult services
