Job Description

An exciting opportunity has arisen In Hywel Dda University Health board for a band 7 Specialist Clinical Practitioner for Frailty. The role will be based in the Llanelli locality and the base for the role will be at Eastgate Offices in the heart of Llanelli.



Eastgate is a very busy open plan office that is integrated and houses a multitude of health, social care and voluntary care agencies.



The role will involve attending weekly MDTs and managing your own caseload of complex patients who are frail. You will be expected to travel between bases and to and from patients/clients' homes.



Support the prevention of unplanned hospital admissions, identifying and initiating Comprehensive Geriatric Assessments where required. and

setting up management plans in collaboration with the patients, carers and community integrated team, to enable the patient to stay at home

where possible, adopting a home first approach.



The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes



Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
