An opportunity has arisen for a dynamic and motivated Specialist Community Public Health Nurse (Health Visitor) to join the Abingdon and surrounding locality for a full time post. The post is to work in the integrated 0-19 Children's and Young People Public Health Service based in Abingdon and surrounding areas. This service brings together Health Visiting, School Health Nursing, and the Family Nurse Partnership, delivering the Healthy Child Programme (HCP) to provide an innovative and high-quality service that helps children, young people and families to thrive.
Health visitors, community public health nurses (previously known as community staff nurses) and community public health associates (previously known as community nursery nurses) will now be working with children up to 8 years of age where there are known concerns or vulnerabilities and will provide health related episodes of care to support with areas such as behaviour, diet, and sleep. There has also been an introduction of a new four-year health and development review to support school transition and address any school readiness concerns.

You will be leading the delivery of the Healthy Child Programme completing the 5 mandated contact with the addition of the new 4 year health and development review to support school readiness and transition. You lead and will be responsible for a skill mix team delivering public health service initiatives to a defined population. You will work as part of a team that includes Community Public Health Associates, and Community Public Health Nurses with additional support from experienced Locality Team Leaders delivering a service alongside SCPHN SHN and the FNP in 0-19 locality teams.
You will deliver services to children and families within homes, and community settings. Clinical activities will include assessing, planning, implementing and evaluating family focused care and delegating to CPHA and CPHN when appropriate. You will develop multi agency relationships and work closely with other agencies including the voluntary sector.
The role will require the post holder to adhere to Safeguarding policies and actively engage in safeguarding processes.
