Job Introduction

We are seeking a compassionate and dynamic Specialist Community Nurse with previous experience working within a Learning Disabilities service to join our small and welcoming multi-disciplinary Learning Disability and Autism team, primarily based in Swindon. This role supports adults with diagnosed learning disabilities whose needs cannot be met through mainstream services.


Some travel across the BSW patch may be required so willingness to travel and a full UK driving license and access to a vehicle is essential.


Recent experience working in a Learning Disabilities service within the last 5 years and confidence working with individuals with learning disabilities is essential.


Main Responsibilities

In the role of a Band 6 Specialist Community RNLD or RMN Nurse (Learning Disability & Autism) you will:

Conduct holistic assessments, risk assessments, and develop crisis plans for individuals referred to the service. Provide tailored interventions, advice, and support to service users, families, and carers. Identify whether service user needs can be met within the team or require referral to mainstream services with reasonable adjustments in place. Manage a caseload, including the monitoring and management of associated risk. Take a lead role in multidisciplinary team (MDT) meetings, interface meetings, and acute case meetings as appropriate. Work collaboratively within a diverse team comprising nurses (learning disability and mental health), occupational therapists, psychiatrists, psychologists, physiotherapists, and administrative staff. Contribute to ongoing service review and improvement, particularly as we transition under the new HCRG Care Group structure.

The Ideal Candidate

Essential

A qualified nurse (RNLD or RMN) with experience in learning disability and autism services and in a community setting Strong assessment and care planning skills Confident in leading meetings and working independently as well as part of a team Committed to promoting person-centred care and accessibility for all individuals Experience in working with clients where behaviours may be challenging Experience of working with clients with complex needs A full UK driving license with access to a vehicle and willingness to travel across the BSW patch Knowledge of relevant legislation and knowledge of the UK health sector and understanding of health care provisions along with strong English language and communication skills Good assessment skills Knowledge of safeguarding Ability to risk assess and care plan Knowledge around non-medical interventions
