Job Description


We are seeking an experienced and enthusiastic Specialist Doctor to join the consultant team in the Hospital at Home service at the Royal United Hospital Bath NHS Foundation Trust. This role will involve delivering exceptional patient care and supporting the development of the service.The focus of the role will be working within the virtual ward, overseeing all aspects of acute care at home to ensure the safe management of patients in their own environment. This will include participating in MDT discussions, providing guidance of medical management, and when necessary, facilitating readmission to the acute hospital.The team has grown rapidly and currently supports 62 beds on the Hospital at Home ward.This post is for a part time Specialist Doctor for Hospital at Home starting as soon as possible and is a permanent term post.Main duties of the jobTo provide expert assistance to the multi-disciplinary team in delivering of a high quality service to patients being cared for under Hospital at Home. The post holder is expected to participate in all aspects of the patient pathway as appropriate and will be expected to perform delegated duties with a high degree of clinical skill and knowledge, working remotely in a home setting.You will be part of the multidisciplinary team arranging treatment plans at home including rehabilitation and discharge. You will be expected to have very good communication skills and communicate regularly with the MDT, patients and their relatives.The post involves working standard days Monday to Friday. Working hours will be 28 across the week working between 08:00 to 18:00.   • Degree --- Full registration and licence to practice with the GMC ----MRCP(UK) or equivalent ---Entry to the GMC specialist register (or expected within 6 months) via CCT, CESR or ECR routes.

  • C.C.T. General Medicine
Clinical ExperienceEssential
  • Clinical training and experience equivalent to that required for gaining UK CCST in Geriatric Medicine
  • Ability to offer expert clinical opinion in the field of Geriatric medicine
  • Experience of Rehabilitation in a variety of settings
  • Ability to take full and independent responsibility for clinical care of patients
  • Demonstrable experience in discharge planning and frailty
Desirable
  • Experience of working with community teams
  • Experience working in acute setting in ED/ Frailty
  • Demonstrates ability to innovate and change current practices to improve the patient experience
