Job Title: Specialist Nurse - Child Weight Management

Location: Medway & Swale - Remote working will be considered

Hours: Part time and Full time (Hours negotiable)

Reporting To: Contract Manager

The Role:

To be specialist weight management nurse for children and families on the tier 3 child weight management service as part of the Multi-Disciplinary Team.

Key responsibilities: -

  • Provide online and face to face 1:1 support for children and families within community, school, or home setting as appropriate.
  • Support group delivery of core curriculum as required.
  • Use psychological coaching strategies, including solution-focussed and motivational interviewing skills to negotiate change to overcome barriers to achieving a reduction in BMI.
  • Be sensitive and supportive to the psychological and physiological needs of participants, and signpost additional support where appropriate.
  • Effectively communicate with clients and relatives, providing support as appropriate in sometimes very challenging and distressing situations, referring to other members of the MDT where necessary.
  • Regularly collect and upload data from service users in a timely and accurate manor. Data collected includes anthropometric and blood pressure measurements and questionnaires to monitor changes in healthy lifestyle behaviours at baseline and at 3, 6 and 12 months.
  • Regularly evaluate the success of the nursing service provision and wider programme and provide feedback to ensure continuous improvement in terms of outcomes.
  • Attend and contribute to multi-agency meetings, bringing together practitioners from different sectors as part of a \'team around the family\' (TAF) approach. This will include attendance of child protection conferences and core meetings.
  • Work in partnership with families and other key people in the young person\'s life, as well as with professionals from other organisations involved with young people such as social care, health, police, education, youth offending teams and local authorities, to build a strong support network.
  • Ensure that our approach is tailored to meet the needs of all individuals, particularly those from groups at risk of health inequalities from example those from BAME communities and those with learning disabilities
