More detail about the role The current vacancy will be based within the Integrated Community Stroke and Neuro Service (ICSS) and provides an opportunity for a part time (22.5 hrs a week) specialist Nursing post. The team support patients after a stroke or other new Neurological diagnosis enabling their care to be transferred from an inpatient environment to a community setting. They enable people to continue to receive high quality, specialist and patient focussed support and rehabilitation in the community.
The team currently consists of physiotherapy, occupational therapy, speech and language therapy, psychology and rehab support workers. We are excited to introduce a nurse post into the team for the first time. The successful candidate will work alongside an established small nursing team in the Eastern locality and will have supervision and support from the Clinical Nurse Manager for Stroke to develop and deliver pathways of care in the Northern locality. The ICSS also work closely alongside short-term services and the local health and social care teams, as well as third sector organisations such as the Stroke Association, to ensure that models of rehabilitation are integrated and patient focussed.
The postholder will provide stroke specific nursing in the community whilst working towards patient centred goals. There will be a combination of community visits, online reviews and group consultations with a focus on working towards providing a nursing assessment for every patient that has had a stroke within the Northern geographical area. This is an opportunity to develop new ways of working to ensure that patients receive specialist support which is based on the most recent national stroke guidelines and which is tailored to the individual. The ICSS is part of a broader Neurorehab Pathway which spans in-patients (Northern), community (Eastern and Northern) and outpatient settings (Eastern and Northern) to provide specialist, evidence-based rehab to patients following stroke or other neurological diagnosis.
This integrated pathway ensures that services are coordinated and support an innovative and dynamic approach to reviewing models of delivery providing services that optimise rehabilitation and recovery opportunities for our patients. You will have the opportunity to develop new skills and expertise as well as be involved in regional and local service improvement projects. The Neurorehab service has a proven track record in development of our staff and opportunities for career development. We have number of senior staff trained to MSc level, and have established advanced clinical practice and consultant AHP roles within the team.
