to join our community team. You will work autonomously as an expert practitioner, managing a complex caseload of patients with COPD, bronchiectasis, ILD and other chronic respiratory conditions.
This role focuses on delivering high-quality respiratory care in the community, preventing avoidable hospital admissions, facilitating early discharge, and supporting long-term condition management.
You will work closely with the Clinical Service Manager, multidisciplinary teams across primary/secondary/tertiary care, and local partners to ensure delivery of evidence-based, safe, and effective services.
Key Responsibilities
Clinical Duties
Manage and triage referrals, independent caseload management, and community respiratory clinics.
Conduct home visits for housebound patients and run nurse-led respiratory clinics.
Assess, diagnose, plan, implement, and evaluate treatment for complex respiratory patients.
Provide specialist interventions including spirometry (ARTP), oxygen assessments, blood gas interpretation, and prescribing (V300).
Act as a key link between acute hospitals, oxygen providers, community teams, and patients.
Support pulmonary rehabilitation and health promotion activities.
Provide expert advice to clinicians, patients, carers, and MDT colleagues.
Deliver telephone-based patient support via the COPD direct access line.
Quality, Governance & Safety
Lead and support clinical governance at team level.
Ensure service compliance with CQC and organisational standards.
Participate in audits, incident investigations, complaints reviews, and service improvement initiatives.
Contribute to data collection, reporting, outcome measurement, and performance monitoring.
Leadership & Education
Provide peer supervision, appraisals, and clinical oversight to nursing staff.
Identify training needs and support the development of the clinical team.
Deliver education to patients, carers, and healthcare professionals.
Participate in MDT meetings, organisational training, and professional networks.
Operational & Collaborative Working
Promote integrated working across acute, primary, and community services.
Support strategic development of local respiratory pathways and services.
Uphold information governance, safeguarding, medicines management, and health & safety standards.
Essential Requirements
Registered General Nurse (NMC)
Minimum
2 years' experience
at Band 6 in Respiratory Nursing
Master's level/postgraduate qualification in Respiratory Care
Advanced clinical assessment skills
Post-registration respiratory nursing course
Non-Medical Prescribing (V300)
or willingness to work towards
ARTP Spirometry or equivalent qualification
Demonstrated experience in managing complex respiratory conditions
Strong clinical leadership experience
Excellent communication, IT, organisational and patient-education skills
Ability to manage high-risk clinical situations independently
Valid UK driving licence & access to a car
Desirable
Community nursing experience
Smoking cessation qualification
Experience with NIV, tracheostomy care, palliative respiratory care
Experience in delivering pulmonary rehabilitation
Teaching/presentation experience
Counselling skills
