In CTM, we're building our services to make a real difference -- and you could be part of it. Our @Home service is based at Dewi Sant Health Park and across RCT and Merthyr, and we are looking for a Band 6 Occupational Therapist to join us.

As an Occupational Therapist, you'll play a key role in shaping how we deliver care. You'll be assessing and supporting individuals in community settings, including their homes, and linking closely with integrated Health & Social Care teams.

Work autonomously across community settings, undertaking lone working in service users' homes



Manage a defined caseload, delivering specialist assessment and intermediate care



Apply evidence-based, person-centred approaches to plan and evaluate interventions



Maintain accurate clinical records in line with professional and local standards



Develop your skills and expertise through a personal development plan



Provide leadership and supervision to new staff and students



Collaborate with a wide multidisciplinary network including hospital teams, care managers, third sector organisations, and primary care



Your impact will be seen as admission avoidance and reduction in hospital stays, while improving independence at home.



This is a full-time role, working Monday to Friday, 8am -4pm ,offering a consistent schedule while making a real impact to support your patients to live as independently as possible.
