Job Description

An Exciting opportunity has arisen to work within the Newly developed Aintree Therapy Home First Team based on the Aintree Site of University Hospital of Liverpool Group. This band 7 Team Lead post will be a dynamic role in providing both Clinical and Non clinical Responsibilities across a 7 day service. There will be an expectation to carry out Clinical care, conducting community working by carrying out assessments with patients within their home environments.



The focus of this team is Discharge to Assess visits. This model prioritizes timely hospital discharge for patients who are medically stable, with assessments for ongoing needs completed after they return home. This "home first" approach aims to reduce hospital stays and promote independence, focusing on providing support and rehabilitation in the community.



You will be working collaboratively with stakeholders such as Merseycare, City council Social work teams to make sure patients can remain at home.



You will lead a team of OTs, Physios, Assistant practioners & Therapy Assistants providing supervision, appraisal, training and service improvement, Any training you require to fulfil this post will be available to you along with regular supervision from a therapy lead



There is an expectation to be flexible over a 7 day service. Flexible working will be considered. Community working experience is desirable. Car driver/access to own vehicle is desirable.



Expert assessment of patients within their home environment to encourage and support timely hospital discharges from the acute trust.



Have a sound knowledge of local stakeholders across the main local boroughs to utilise to maintain patients safety and well being within their home environment.



Audit of the service against local and national standards in close partnership with the management team



Training and support of junior staff members



Senior oversight of a therapy team including aspects such as performance reporting, sickness management and supervision



Participation in national audit and research projects



Ensure adherence to governance processes including datix and complaint investigations



Support staff with health, wellbeing and sickness management
