Specialist Pharmacist Critical Care
Job Description
To be responsible for providing and overseeing the pharmacy clinical service provided to the Critical Care wards within the hospital Carry out education and counselling of new patients at the start of newly started high risk medicines. Ensures the provision of a proactive, high quality, cost effective, safe and responsive service to Critical Care wards. To actively develop the Critical Care pharmacy service, exploring opportunities for clinically advanced services. To provide a clinical ward pharmacy service to the Critical Care wards and lead on the education and training of other members of pharmacy and MDT in the pharmaceutical management of these patients.
Check the clinical accuracy and appropriateness of prescriptions for patients To ensure the appropriate ordering of medicines for Critical Care to ensure medicines are available for patients and to minimise wastage from unused or expired medicines. Attend the board rounds and MDTs where applicable within Critical Care, providing expert pharmaceutical advice. To actively lead on efficiency savings within Critical Care. To be the pharmacy department liaison with the wider team within Critical Care.
To identify and implement opportunities for the efficient use of medicines in Critical Care. To support with the late night, weekend and bank holiday rota as part of the departments 7 day services. To develop a personal audit and research portfolio.
