This is a very exciting opportunity for an experienced Physiotherapist to support the development, implementation, continuous monitoring and improvement of the Active Together Programme for a 12 month fixed term contract to cover maternity leave.
The role of Specialist Physiotherapist is to help deliver and sustain the success of the exercise element of the Active Together Programme at Airedale, working collaboratively with the Clinical Specialist Physiotherapist (Band 7) and Operational Manager. The Active Together programme runs alongside the existing diagnostic and treatment pathways to ensure patients are optimally prepared for treatment and beyond. It aims to maximise resilience to treatment, improve longer term health, minimise side effects and empower patients to be partners in their healthcare.
The programme combines physical activity, nutritional support and psychological support alongside the clinical pathway. This is a funded 5-year staged implementation programme. Patients receive individual assessments and are triaged into a support pathway suitable to their needs. This is delivered within locality or selected facilities closest to the patient's home or may be a self-management pathway.
You will be joining a small friendly team based at Skipton Hospital consisting of the Operational Manager, another Physiotherapist, a Dietitian, a Psychologist, and plans to recruit a Project Co-Ordinator, and Fitness Instructors in the future.
Support the implementation of the Active Together prehabilitation and rehabilitation services at Airedale NHS foundation trust.
To frequently undertake the comprehensive assessment of patients, with a complex presentation and multiple comorbidities.
To analyse the assessment to formulate individualised, evidence-based treatment plan, using own clinical reasoning and a range of treatment skills and options to formulate a specialised programme of care. Prioritising this efficiently considering clinical need, priorities and time management.
Be an integral part of the Cancer Prehabilitation team working within the MDT to deliver a high-quality effective service, including identification of dietetic and psychological needs of these patients, liaising with the appropriate teams when required.
