This is an exciting opportunity to join an Award Winning multidisciplinary team.



The Elderly Virtual Ward Team deliver care closer to home, interacting with the Consultant Geriatrician, therapist, nursing and social care team.



The post holder will deliver seamless care across organisational boundaries reviewing patients in a community setting in order to improve the ability for patients to self-care; to enable patients to recover at home following illness/ preventing the need for patients to be admitted to hospital.



We are looking for an experienced physiotherapist who is highly motivated and a self-starter with the ability to work as part of a multidisciplinary team and provide skills, knowledge and expertise in the care and management of frail older patients in a community setting. We provide a comprehensive assessment and rehabilitation programs for patients in their own homes and respond to the needs of the frail elderly population with a wide range of conditions including complex co-morbidities, frailty, respiratory, musculoskeletal, and orthopaedic.

It is essential that you have a full UK driving licence and have a car available for work
