In this role you will be at the forefront of delivering new models of care through transformation that meets the needs of our community, and will be able to contribute to the creation of what that looks like as well as positively impacting and enhancing the experiences of our patients in the diverse community of Brent.You will provide day-to-day specialist clinical care in a supported environment and within a dynamic team, to a diverse group of patients.
