Job Description

To work as an autonomous Specialist School Nurse to deliver the Healthy Child Programme and to support the health and emotional well-being needs of Children and Young People (CYP) aged 5-19 who are Educated Other Than at School, including CYP who are attending pupil referral units, electively home educated (EHE), missing from education and who are open to the local authority attendance team and assessed as having a identified health need requiring school nursing intervention. To be the allocated named School Nurse for three Alternative Provisions (AP) within Southend-On-Sea. To work in partnership with a wide range of professionals including but not limited to, GPs, Health Visiting, School Nursing, Community dental, Community paediatricians, Mental health services, Open Road, Supporting Families, Social Care, and the Youth offending team to ensure that children and young peoples health needs are supported, and safeguarding is maintained. To engage with safeguarding processes to ensure concerns are escalated through appropriate channels and a suitable plan of support can be developed to meet the need of the Child/Young person.



This includes adhering to the local SET and national safeguarding policies and procedures, accessing mandatory safeguarding supervision, liaising with the locality safeguarding team and social care, writing safeguarding reports and attending Child Protection conference, core groups and Child in Need meetings. Completion of health needs assessments to support in the identification of health needs and inform a personalised plan of care/interventions in collaboration with the child, young person, and their parent/carer. To establish and maintain relationships with voluntary agencies to support the understanding of health needs, increase accessibility to services and address inequalities. To support effective service responses and pathways where there are multi-agency service interfaces required.



To maintain an excellent standard of records for the service. It is recognised that children and young people within this client group often present with complex and/or enduring needs which require a multidisciplinary and flexible approach in the aim of recognising and addressing barriers to services and reducing health inequalities. Due to these factors, children and young people in this client group may be at increased vulnerability. To promote to and upskill the Southend Children, Young People & Family Public Health service (0-19 team) and other agencies regarding the health needs for this client group including health inequalities and interventions.
