This role involves delivering specialist assessments and interventions for adults with speech, language, communication, and swallowing disorders within a community setting. The postholder will work autonomously and collaboratively across GP practices, care homes, and within patients' homes. Responsibilities include differential diagnosis, treatment planning, discharge, and support for junior staff. You will also participate in service development, training delivery, and quality improvement initiatives.

Welsh Skills Desirable:

This post is advertised as Welsh Desirable. This means Speaking & Listening at Level 3 (equivalent to CEFR B2). We don't need fluency. Level 3 means basic conversations with patients about their everyday health. For more information, see the 'Croeso Candidate' booklet below.

The Specialist Speech and Language Therapist (Band 6) will work as an autonomous practitioner within the Integrated Community Network Team in Bridgend, providing high-quality, person-centred care to adults with speech, language, communication and swallowing disorders. The post holder will collaborate across GP practices, care homes, and community settings, responding to patients' needs in a timely and proactive manner to prevent unnecessary hospital admissions and support independent living.



As a key member of a growing multi-disciplinary team including social workers, nurses, occupational therapists, physiotherapists, pharmacists, and others, the therapist will contribute to a holistic model of care that prioritises choice, control, and patient voice. Duties include specialist assessment, differential diagnosis, treatment planning, discharge, and the delivery of evidence-based interventions for a diverse caseload.



The post requires excellent clinical decision-making, the ability to manage risk, and the flexibility to meet urgent service needs. The therapist will manage their own caseload, contribute to service development, and support junior staff and student placements. They will engage in multidisciplinary meetings, provide specialist advice to patients and colleagues, and lead or participate in training, supervision, and quality improvement initiatives.

Key responsibilities:

Autonomous management of a complex caseload across home, clinic, and care home environments. Delivery of specialist SLT services, including assessment, differential diagnosis, and treatment. Implementation of personalised care plans, ensuring safe and effective intervention for adults with communication and swallowing difficulties. Communication of complex clinical information to patients, carers, and professionals in sensitive and accessible ways. Coordination of timely referrals and engagement with wider healthcare and social care professionals. Contribution to audits, research, service improvement, and clinical governance processes. Provision of clinical support and supervision for less experienced therapists and students. Use of alternative and augmentative communication systems where appropriate. Participation in health promotion and community awareness activities. Maintenance of high clinical and professional standards in line with HCPC and RCSLT guidance.

The therapist will be expected to demonstrate advanced auditory and perceptual skills, emotional resilience, and the ability to manage challenging behaviour and unpredictable working environments. The post involves a balance of independent clinical work and collaborative team engagement, with opportunities for continuous professional development and innovation.
