T he Older People's Mental Health Community Mental Health Team w o r k c l o s e l y w i th GPs to ensure early i d e n t i f i c a t i o n o f patients with mental health needs and facilitate timely referrals. T h i s i n t e g r a t e d a p p r o a ch to t h e c a r e o f the elderly a n d th e s u p po r t o f t he i r c a r e r s, w il l b e p r o v i de d i n pa r t ne r s h i p w i th l o c a l h e a l t h c a r e , s o c i a l c a r e an d v o l un t a r y/ i nd e pe n d e n t o r g an i s a t i on s.



Assessment and treatment of functional and organic psychiatric disorders in the community as well as in Nursing/ Residential Homes in Bexley.

Responsibility for the continuing care dementia units in Bexley with regular input and reviews.

Facilitate early discharge planning and effective use of the inpatient units' beds

Preparing papers and presenting to the Multidisciplinary panel for placements in residential and nursing homes for older people known to the team

Input to Multidisciplinary panel discussions for placement reviews in residential and nursing homes for older people known to the team

As it is an integrated team (integrated with social services) there is social care input to the patients known to our team with regards their care packages, respite care and placements via the MDT panel.
