The Enhanced Support and Liaison Service (ESLS) is a newly formed service that supports adults with learning disabilities and/or autistic adults, who are:



at risk of placement breakdown at risk of hospital admission and/or need support to access mainstream mental health services.

This post would suit a Speech and Language Therapist who enjoys fast-paced, diverse clinical work, and can work well within a team/as well as autonomously. The post provides an ideal opportunity for a Speech and Language Therapist to consolidate their skills with the opportunity of working across our three service pathways. Equally, this would be a great opportunity for someone who is newly qualified, who would be interested in the opportunity of a clearly planned Band 5 - 6 development post.



You will be responsible for the assessment, planning and implementation of evidence-based care to service users with learning disability and / or autistic individuals who have communication needs and dysphagia.



You will provide professional advice and support across the three pathways within ESLS on communication and dysphagia related topics. The three pathways you will work are:



Community Liaison Pathways Inpatient/discharge Pathway Enhanced and intensive community Intervention & Support

ESLS work with teams and services across the trust to provide support and intervention for those who are currently resident within Mental Health inpatient services, who are at risk of admission to these services or who are at risk of placement breakdown. This support is primarily through liaison, advice, training and consultation with other trust services and with wider system partners including the Integrated Care Board (ICB), Lincolnshire County Council, United Lincolnshire Hospitals Trust, Police and Voluntary and Third Sector Organisations.

To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.



To delegate aspects of the patient treatment plan to intervention workers, whilst maintaining overall responsibility for patients care, where appropriate.



To manage case-load and service delivery in accordance with and in order to contribute towards the attainment of Service/Trust productivity requirements.



Ensure up-to-date agreed care plans, risk assessments and reviews are in place.



Work with individuals and their carers to identify their related needs and circumstances; and enable them to understand, manage and where appropriate change their behaviour.



Plan, implement, review and improve interventions to meet people's identified needs and manage their inherent risk.



Protect people from abuse, neglect and harm.



To work collaboratively and promote effective working relationships with members of the multi-disciplinary team, ensuring effective and appropriate clinical decision-making, both within the Enhanced Support and Liaison Service and with external agencies.



Provide management supervision to junior staff in line with the Trust policies and protocols for supervision, appraisal and mandatory training.
