Job Description

To ensure that all records are maintained and stored confidentially and in line with GDPR requirements To work as part of a multidisciplinary team member To Follow Castel Froma Neuro Cares policies and procedures Prioritising workload through demonstration of high level of understanding of clinical needs and risks to individuals to ensure residents psychological, social and physical needs are met Communicating effectively and sensitively with residents, family, and carers as well as members of the clinical team and therapy services Effectively communicate complex information to residents and family members regarding SALT interventions Supporting residents with discharge home and have effective communication with external agencies to ensure the process is fluid Maintaining your own continuous development professional practice, participate in performance and development reviews To contribute to and participate in identified audit and service development projects
