Job Description

Acute Medicine in Hereford is at the forefront of improving the patient experience and journey through the hospital. Acute Medicine is responsible for the Acute Medical Unit (a 24-bedded inpatient unit), Same Day Emergency Care and the acute on call service. The recently opened Acute Medicine virtual ward represent further recent investment and innovation led by the acute medical team. The Acute Medicine team work within the broader acute floor and works closely with the Emergency medical team and the broader unplanned care service across the county.



The Acute Medical service is well supported by Hospital at Home (providing outpatient intravenous therapy), a GP pathway for DVT, Clinical Nurse Specialists in Cardiology, Respiratory, Stroke, Diabetes, Oncology and Gastroenterology. The Acute Medical team consists of junior doctors (both training and non-training grades), Advanced Nurse Practitioners and Physician Associates (PA).



Hereford County Hospital has an average of 40 medical admissions per day.



This is an exciting time in Medicine in Hereford with big plans for change and improvement.



Please be aware that this is a fixed term post for 6 months only. The successful candidate will be expected to start on 6th August 2025 and remain in post until 03 February 2026.

This is a post is based between the Acute Medical Unit and Same Day Emergency Care (SDEC). The post holder will be clinically supervised by the Acute Physicians with overall accountability to the Lead for Acute Medicine and Service Unit Director for Urgent Care. Hereford provides an excellent broad based training in general medicine within a very friendly and supportive department. The weekly timetable for this post is indicative only, and will be adjusted to suit the educational needs of the post holder and any changes in the departmental service arrangements.

Worcestershire is our neighbouring county. The post holder will be required to travel between sites.

The Acute Medical Unit (AMU) and Same Day Emergency Care (SDEC) are being further developed to reshape the flow of acute admissions through the hospital with more emphasis on senior front of house decision making, early diagnostics and assessment and admissions avoidance. The rota will be discussed at interview and finalised on appointment. In addition, the post can be adapted to suit the successful candidate.



You will be working on the on-call rota on a 1:18 basis. Your average weekly hours will be 45 : 15, doing a mixture of on-calls and night shifts.
