TheStaff Nurseis a registered nurse working as part of the Health Visiting Team providing preventative and supportive health care to children and families in Surrey.This includes the identification, monitoring and provision of targeted support to vulnerable children and those whose welfare and safety may be at risk.OurStaff Nurseswork as a member of the Health Visiting Team maintaining a public health focus under the supervision of a Specialist Community Public Health Nurse (SCPHN) to deliver the Healthy Programme in a variety of settings including the home and community venues as delegated by the Health Visitor. All allocated work is completed within the designated competency framework.BaseOur Healthy Family Teams support our Surrey-wide service, and you will be based at one of our Office Hubs across Surrey with the opportunity for hybrid working.HCRG Care Group is a flexible-first employer, whilst you will have an office base, our teams are agile and work in flexible/hybrid arrangements.Main duties of the jobAs aStaff Nurseyou will play a vital role in improving health and wellbeing outcomes for children, young people, and their families. Working within a Healthy Family Team, youll provide compassionate, outcome-based support in homes, schools, and family hubs across Surrey. Your Responsibilities Will Include:Work within the boundaries of the Staff Nurse Competency frameworkWork with the Health Visiting Team to achieve Key Performance IndicatorsWork in partnership with children and families to identify any health needs at the earliest opportunity and to provide appropriate support to achieve positive outcomes.To work in partnership with Early Years Services in the community such as Childrens Centres and Early Years settings.To support Infant feeding and parent infant relationships by adhering to the Baby Friendly Initiative StandardsTo complete the Family Health Needs assessment for allocated mandated reviews and update the SCPHN (Health Visitor) with any areas of concern.To provide support to families using the Solihull Foundation Approach model and Five to Thrive trauma-informed practice in behavioural management, parenting skills and in the delivery of programmes of care to vulnerable families as identified by the SCPHN Health Visitor.To provide ongoing support and intervention to allocated families and updating the SCPHN health visitor when significant changes occur.Please see attached job description for a full list of responsibilitiesAbout usAbout the CompanyWe change lives by transforming health and care.Established in 2006, we are one of the UKs leading independent providers of community health and care services, working with health and care commissioners and communities to transform services with a focus on experience, efficiency and improved outcomes.
