You'll also collaborate closely with our Citywide Services, including the Falls, Continence, Urology and Colorectal Care, and Palliative and End of Life Care teams, as well as GP services, ensuring the best possible coordination and continuity of care for patients across West Leeds.
Key responsibilities include:
Providing safe, evidence-based clinical care in patients' homes.
Maintaining excellent communication with patients, families, and colleagues.
Delivering high standards of nursing care and supporting patients to achieve their personal health goals.
Promoting self-management and independence wherever possible.
Demonstrating resilience, adaptability, and motivation in a dynamic community setting.
Upholding clinical governance standards and contributing to continuous service improvement.
Championing patient safety, dignity, and quality at every opportunity.
