Job Description

As a registered nurse the post holder will provide care to elderly with acute health needs in the ward. The post holder will be accountable and responsible for providing a high standard of patient centred care. To carry out care with direct supervision and support from the team. To communicate effectively within the multidisciplinary team. Contribute towards the effective running of the department including patient safe and timely care.



Conduct comprehensive assessments to identify frailty and related health risks in older adults.



Develop, implement, and review individualized care plans tailored to the needs of frail patients.



Coordinate multidisciplinary care involving doctors, therapists, social workers, and caregivers.



Provide education and support to patients and their families on managing frailty and associated conditions.



Monitor patients' progress and adjust care plans as necessary to prevent hospital admissions or complications.



Facilitate access to community resources and services to support patients' well-being.



Maintain accurate and confidential patient records in compliance with healthcare standards.



Promote best practices in frailty management and contribute to continuous service improvement.



Participate in training and professional development related to geriatric care and frailty.

The Frailty Nurse specializes in assessing, managing, and supporting older adults who are living with frailty. This role focuses on preventing deterioration, promoting independence, and coordinating holistic care to improve patient outcomes in clinical setting.

Key Responsibilities:

Conduct comprehensive assessments to identify frailty and related health risks.



Develop and implement individualized care plans tailored to the needs of frail patients.



Monitor patients' physical and functional status, adjusting care plans accordingly.



Collaborate with multidisciplinary teams including doctors, therapists, social workers, and caregivers.



Provide education and support to patients and families on managing frailty and maintaining wellbeing.



Facilitate access to community resources and services to support patients' needs.



Promote best practices in frailty management and participate in continuous service improvement.



Maintain accurate, confidential patient records in line with healthcare standards.



Support the prevention of hospital admissions and readmissions through proactive care.

Qualifications:

Registered Nurse (RN) with experience in geriatrics, community, or primary care nursing.



Specialized training or certification in frailty assessment and management preferred.



Strong clinical and assessment skills with a holistic approach to patient care.



Excellent communication and interpersonal skills.



Ability to work independently and as part of a multidisciplinary team.



Compassionate and patient centered approach.
