Purpose
To deliver high-quality, person-centred nursing care to residents in accordance with their individual care plans, ensuring clinical excellence, compliance with the NMC Code, and alignment with CQC Fundamental Standards of Care.
Key Responsibilities
Clinical Care Delivery
Assess, plan, implement, and evaluate care for residents, ensuring care is evidence-based and reflective of individual needs.
Administer medications safely and accurately, in line with legal and professional guidelines.
Monitor and respond to changes in residents' health and well-being, escalating concerns appropriately.
Provide support with personal care when needed, ensuring dignity and respect are maintained.
Clinical Records and Documentation
Maintain accurate, up-to-date care plans and records using the home's care management system.
Document nursing interventions, risk assessments, and outcomes clearly and professionally.
Ensure completion of required documentation such as MAR charts, wound care logs, and repositioning charts.
Compliance and Governance
Work in line with CQC regulations, NMC Code of Practice, and internal policies.
Participate in audits, medication reviews, and clinical supervision processes.
Report incidents, accidents, and safeguarding concerns in a timely and professional manner.
Teamwork and Communication
Work collaboratively with care assistants, nurses, visiting professionals, and family members to ensure coordinated care.
Act as a mentor and role model to junior staff, providing guidance and support.
Participate in shift handovers and contribute to multidisciplinary reviews.
Resident Engagement and Experience
Promote independence, choice, and dignity in all interactions with residents.
Build trusting relationships with residents and their families.
Contribute to a positive, homely environment and support resident activities when appropriate.
Person Specification
Essential:
Registered Nurse (RGN, RMN or RNLD) with active NMC PIN
Good clinical knowledge, including medication management, wound care, and chronic disease monitoring
Excellent communication, assessment, and organisational skills
Ability to work independently and as part of a multidisciplinary team
A genuine commitment to person-centred care and resident well-being
Desirable:
Experience working in a care home setting
Training in dementia care, palliative care, or other specialisms
Competence using digital care systems
