Job Description

We are looking for a registered nurse who is looking for a new challenge on a busy, exciting acute stroke ward . This is a wonderful opportunity to be part of our Stroke pathway here at NGH and be involved in making a difference to our patients and our service.



The successful candidate will have to build a working relationship with not only the nurses, HCA's, medical team on the stroke ward but also with the wider MDT (physiotherapist, occupational therapist, SALT and dietician) to ensure a smooth pathway for stroke patients.



The Division has a dedicated practice development team who works across the Division and support our nurses within their clinical areas to grow and develop. Staff have an opportunity to undertake bespoke modules which are specific to their own clinical specialty.



We are looking for highly motivated and enthusiastic individuals. You will need to be innovative and adaptable, keen to learn new skills proving self-development and be a positive member of our busy team.

Clinical Excellence



The post holder will be responsible for assessing, planning, implementing and evaluating patient care, maintaining at all times the highest standard of nursing care.



Liaise with, and provide relevant information to other members of the multidisciplinary team.



Assist in the supervision and monitoring of nursing practice under the guidance of the nurse in charge.



Work collaboratively and co-operatively with others to meet the needs of patients and their families.



Demonstrate good standards of practice with regard to documentation in-line with Trust policy.



Demonstrate a good knowledge base of general nursing care using the latest evidence on which to base their practice.



Demonstrate competence, or willingness to develop competence, in expanded role skills.

Main Duties and Responsibilities



Patient/client care:



Assisting and contributing in the management and organisation of a ward / clinical area.

Actively contribute to setting and maintaining high standards of quality nursing care.

Actively participate in the education, development and supervision of other staff members.

Act as a clinical and professional role model, assessing and supporting students and learners on placement within the clinical area.

Work collaboratively and co-operatively with others to meet the needs of patients and their families.

To maintain accountability for assessing, planning, implementing and evaluating programmes of care.

To co-ordinate the patients' discharge arrangements in accordance with NGH Discharge Policy.

To educate patients and their carers as required where a need has been identified.

To complete patient documentation correctly.

To maintain a safe ward environment.

To assist in establishing and monitoring protocols/care pathways.

To participate in the promotion and maintenance of effective communication written and verbal.

To ensure patient confidentiality.

To supervise junior staff in the delivery of care.

Responsible for the safe supply, storage, administration, checking and disposal of medications including controlled drugs.

To undertake any expanded roles deemed necessary for the role following the Trust policy 'Expansion of Professional Practice for Registered Healthcare Practitioners'.



Policy/service development implementation:



Undertake the Trust Corporate and Directorate specific Induction and competency Programmes.

To contribute to annual appraisal and be responsible for own personal performance/development plan to achieve KSF outline for role.

Able to identify learning needs in self, negotiating a Personal Development Plan recognising own limitations.

Contribute to the education and training of others, gaining an understanding of clinical supervision.

Actively promote evidence-based practice within the clinical area and contribute to audit initiatives.

Adhere to all Trust Policies and Health & Safety, as applicable.



Financial & physical resources:



To develop and maintain an awareness of budgeting, exercising care and economy in the ordering and use of equipment, supported by written documentation where applicable.

Responsible for the effective and efficient use of Clinical, physical and financial resources.



Human resources:



To help maintain a suitable learning environment for staff and act as a facilitator/mentor/preceptor in the supervision and teaching of less experienced staff.

To attend Trust/local orientation programmes and mandatory training sessions.

To be responsible for developing and sustaining own knowledge, clinical skills and professional awareness.



Research & development:



To practice clinical competencies in line with relevant Trust initiatives e.g. Essence of Care.

To maintain and promote an awareness of evidence-based practice and participate in its Implementation.

To contribute to research and development programmes within the ward/department.

Use health related information only for the purposes of what it is intended for and in accordance with the Data Protection act.

Ensure confidentiality is maintained at all times and information relating to patient / users and personnel is used only in connection with authorised duties.
