Job Description

This role is eligible for visa sponsorship under the UK Visas & Immigration (UKVI) Skilled Worker route however sponsorship can only be provided where applicants meet the specific requirements set by UKVI, including the relevant skill and salary thresholds (25000), and any role-specific criteria.



The post holder will be responsible for the smooth and efficient running of doctor and nurse-led clinics and the delivery of skilled nursing interventions to outpatients attending for day case procedures. They will be responsible for the assessment of patients' care needs and the development, implementation, and evaluation of programmes of care without supervision.



To deliver safe, competent, and compassionate care to renal patients attending clinic reviews and day-case procedures



To undertake haemodialysis for those with end stage kidney disease



To work unsupervised, as an effective, flexible team player



To deputise in the absence of senior staff to ensure continuity of care for patients attending the Renal Outpatients department.



To run and support a variety of medical and nurse-led clinics



To assess patients holistically and refer to the multidisciplinary or community teams as required.



To delegate appropriately and act as an effective and professional role model within the nursing team

Plans and delivers care and investigations to renal patients using competent skills in such areas as venepuncture, cannulation, administration of intravenous drugs, ECGs.



Co-ordinates clinic activity and reviews patient care needs within the department, supporting the Healthcare Assistants running clinics. Interprets test results accurately leading to appropriate actions and referrals to the multidisciplinary team as appropriate.



Is actively involved in the development, implementation, and evaluation of programs of care with a flexible approach, involving all members of the multidisciplinary health care team thus promoting patient-centred care.



Provide follow-up care via phone, for patients and carers, keeping them informed and supporting them while at home.



Provides emotional and psychological support to patients and families coping with diagnosis and life-changing decisions on renal treatment and dialysis, referring to other professionals and agencies as needed.



Enables patients and their families to understand renal failure and its treatment, and to gain access to further psycho-social support as required. Takes nursing lead in the care of patient groups with specific needs, such as people with a kidney transplant, vasculitis, diabetes, renal anaemia, etc, monitoring their test results and clinic follow-up by phone and in nurse-led clinics.



Gives pre and post-procedure care to renal outpatients attending day procedures and intravenous infusions, monitoring and acting on their vital signs, escalating as indicated to the medical team.



Actively participates as an effective team member, contributing to the efficient running of the department. Able to transfer patients between departments safely and appropriately according to Trust guidelines and internal operating policies, e.g. managing day case conversions to inpatient care safely.
