Malvern Community Hospital supports patients as a step down to receive ongoing medical treatment, rehabilitation, discharge planning or end of life care which no longer requires acute support but cannot be managed in the community. ANPs and GPs cover the hospital allowing for provisionof care for local residents also. Within this role, you will be caring for a caseload of patients with varying needs, requiring dynamic practice and supporting professional experience and development. We feel passionately about patient care, promoting a holistic and compassionate experience for those we treat. We have strong links with community teams and resources that promotes home first. Main duties of the job
As a Staff Nurse you will be responsible for providing care to people admitted to our ward for rehabilitation. We work closely as a multi-disciplinary team. Our focusispromoting the comfort and wellbeing of patients, enablingthemto live as independently as possible. Malvern is a small but friendly team to work with. As a team we encourage ongoing learning and development through reflection and clinical supervision which we try and embed in our daily practice. Our nurses are required to facilitate the day to day running of the ward, administering medications, working alongside all members of the MDT as well as patients, their families and carers to provided 24/7 rehabilitation to enable the highest level of independence and quality of life. Our nurses are required to attend meetings as needed such as MDT and CHC or Discharge Planning.

7. In collaboration with other team members organise the reception and discharge of patients ensuring effective communication in transfer of information.
8. Carry out delegated nursing care.
9. Use and participate in research / audit in care delivery.
10. Adhere to and promote all clinical practice, health and safety and corporate Trust Policies and Guidelines.
