An exciting opportunity has arisen for a Staff Nurse to join our growing team on a full time, fixed term basis. Wakefield Hospice is looking for those of you who have the passion and drive to deliver high quality specialists palliative care in our inpatient unit, have effective communication skills and at least 12 months UK post-registration experience in working in community care settings. You will be provided with a full induction programme, a buddy/mentor, and general support to allow you to keep developing your skills and knowledge in specialist palliative care whilst working in a high quality clinical environment.

The successful candidate must demonstrate compassion, commitment, caring, competence, quality and safety in delivering person centred care.

Minimum Qualifications:



Registered Nurse level 1 with twelve months UK post qualification experience in an adult NHS setting. Effective communication skills

Job Summary:



Wakefield Hospice provides specialist palliative and end of life services 24 hours a day, 7 days a week. You will be required to work a variety of shift patterns across 24hrs, including rotation to day and night shifts (and on occasion to support the Day Therapy Services) to:

Deliver high quality Specialist Palliative Care and End of Life Nursing as part of the Inpatient Unit multi professional team. To assess, plan, implement and evaluate care for a named group of patients ensuring a holistic approach that is sensitive and responsive to patient and family's needs. To demonstrate compassion, commitment, caring, competence, quality and safety in delivering person centred care. To be responsible for the coordination and smooth running of a shift when required, ensuring it is well organised and safe. As a registered, accountable and autonomous nurse act in the best interests of patients at all times, taking personal responsibility for own actions and decisions. To promote a positive and professional atmosphere in the work place, ensuring Wakefield Hospice's Mission is reflected in practice.

Duties And Responsibilities

Some tasks may be delegated but the post holder remains responsible for ensuring that they are carried out satisfactorily.

Clinical

Assess, plan, implement and evaluate person centred nursing care for a named group of patients in compliance with hospice policy and protocols. Undertake holistic assessment of patients, families and carers, including their emotional, physical, spiritual, practical, welfare rights and as appropriate pre and post bereavement needs. Ensuring that patients, their families and carers are involved in their plans of care. Actively support and plan the discharge planning and process. To develop, update, monitor and evaluate patient care plans ensuring they are person centred and support individual needs. To demonstrate the skills and knowledge that enable the delivery of basic and complex specialist nursing care interventions that are responsive to individual need. Have knowledge of common symptoms, anticipate change and evaluative interventions. To communicate professionally, sensitively and effectively with patients, carers, families, the multi professional team and all staff within the hospice. To deliver concise and accurate patient presentation in daily hand overs and to attend and participate the weekly MDT meeting in the absence of senior nursing team if requested. In the role of ward coordinator be responsible taking a nursing lead on admission and discharge planning as required. Take day to day responsibility for planning and organisation of the shift and prioritise effectively according to patient need, safety and staffing. To communicate effectively with own team and maintain effective communication between the MDT and external agencies. To communicate highly sensitive information, ensuring patients, their families and carers are suitably supported. To respond to helpline calls effectively and in a timely manner, including escalation to senior team members or appropriate signposting or advice is given and documented. To promote and support Advance Care Planning. Ensure accurate and timely records of care (electronic and paper) are maintained, upholding compliance with information governance, being responsible for having a SystmOne card on all shifts. Follow all agreed policies for manual handling, infection control, health and safety and security. Failure to adhere to these guidelines causing harm to a patient, family member or hospice employee could lead to disciplinary action. Action the agreed policy in the event of a fire/fire drill. Ensure the dignity, privacy and confidentiality of all patients and families according to Caldecott principles, hospice policy and guidelines. Participate in audit and evaluation of clinical practice and lead on delegated areas of clinical development. Actively promote and engage in reflection and clinical supervision Actively promote a culture that supports effective team work. Support all students including nurse associate trainees.

Managerial

Prioritise, organise and delegate workload within the team. Participate in Individual Performance reviews for HCAs and or support junior staff in reaching their goals alongside the senior nursing team. Participate in and encourage others to attend team/ward meetings and incident reflection. Deputise for the ward sister /senior staff nurses on occasion, acting as ward coordinator ensuring the running of the ward is safe, effective and cohesive. Support the sister/senior staff nurses ensuring effective staffing levels are maintained to meet patients' needs through planned co-ordination of annual leave, study leave and using bank staff appropriately to cover unplanned absence. To ensure that all complaints and compliments are sensitively listened too and manage complaints and incidents in the absence of the ward sister/senior staff nurse and to escalate according to policy. When coordinating the ward to be responsible for managing any service enquires, triaging help line and out of hour's calls and referrals, informing and discussing with any relevant professionals and taking actions accordingly.

General

Maintain a professional approach to every one you encounter in the hospice and act as an exemplary role model / champion for the hospice mission and values. Support the off duty, annual leave management and sickness reporting process in accordance with policy. Where appropriate represent clinical services in partnership with the hospice fundraising team. Be aware of the importance of efficiency and cost effectiveness within the hospice and act accordingly Support ward events as appropriate to enhance patient experience and quality of life. Ensure all practice is undertaken in accordance with requirements of The Nursing and Midwifery Council.

Education

Attend all mandatory training as requested Ensure requests for revalidation sign off are requested of the ward sister /senior nursing team according to policy. To keep abreast of current developments, advances, guidance and research in palliative and end of life care To develop knowledge and skills in line with agreed competency frameworks. Undertake continuous professional development seeking opportunities to enhance skills and embed lifelong learning in clinical practice. To deliver informal and formal training and education to small groups of staff on agreed topics. To participate in mentorship of nursing students and/or other health care professionals on placement within the hospice. Contribute to the promotion of a positive learning environment within the clinical area
