Your key responsibilities will include:
- Supporting young people in their transition from care to independence, providing practical and emotional support.
- Collaborating with multi-agency teams to develop and implement effective support plans tailored to individual needs.
- Conducting assessments and reviews to monitor progress and adapt support strategies as necessary.
- Facilitating access to education, employment, and housing resources.
- Providing guidance on life skills, financial literacy, and personal development.
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Registered with Social Work England.
- Experience working with adolescents or young adults in a support capacity.
- Strong communication and interpersonal skills, with a compassionate and empathetic approach.
- Ability to work collaboratively within multi-disciplinary teams.
