Job Summary
We are seeking a dedicated and patient-focused Services-Oriented Pharmacist to join our team in Silverdale. This full-time role goes beyond traditional dispensing duties, emphasizing the delivery of clinical services, patient education, and collaborative care. The ideal candidate will be passionate about improving health outcomes through personalized pharmaceutical care and proactive engagement with patients and healthcare providers.
Key ResponsibilitiesClinical Services Delivery
Provide medication therapy management (MTM), chronic disease support, and health screenings.
Administer vaccinations and other pharmacist-led clinical services.
Conduct patient consultations to optimize medication use and adherence.
Patient Engagement & Education
Educate patients on medication regimens, side effects, and lifestyle modifications.
Support self-care and preventative health initiatives.
Collaboration & Communication
Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated care.
Participate in multidisciplinary team meetings and contribute to care planning.
Operational Excellence
Ensure compliance with legal, regulatory, and professional standards.
Maintain accurate patient records and documentation of services provided.
Support continuous improvement in service delivery and patient satisfaction.
Innovation & Development
Contribute to the development and implementation of new pharmacy services.
Stay current with clinical guidelines, pharmaceutical advancements, and service models.
Qualifications
MPharm or equivalent degree in Pharmacy.
Registration with the relevant pharmacy regulatory body.
Proven experience in delivering clinical pharmacy services.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Commitment to patient-centered care and continuous professional development.
Desirable Skills
Certification in immunization or independent prescribing (if applicable).
Experience with digital health platforms or electronic health records.
Knowledge of public health initiatives and community outreach.
