SUMMARY OF ROLE



To provide a personalised and need led service to adults with a Mental Health, Autism and/or learning / physical disabilities by adopting a person-centred approach that fosters self-awareness, personal growth and gives each person the strongest voice with regards to decision making and lifestyle choices.

MAIN RESPONSIBILITES, TASKS & DUTIES



Coaching, Supervising, Motivation & Mentoring



To Provide regular Supervisions to an appropriate number of staff members as appropriate to meet individual, local area, and statutory requirements.

Provide direction, supervision, mentoring, coaching & motivation to the team, always acting as a positive role model to support team members with-in an identified area of the service.

To monitor and review Care/Support plans / Risk assessments and ensure they are relevant, appropriate, and up to date.

To ensure that staff are trained for their roles and responsibilities and that all staff team members within the team have access to and complete appropriate training and learning opportunities.

To establish and maintain effective two-way communication (Supervisions & Appraisals) to ensure that all staff are aware MAK CC policies/procedures and guidelines. Ensuring compliance with Care Quality Commission (CQC), Service Commissioners and Company standards are always maintained.

To provide a confidential, supportive, and nurturing opportunity to reflect on staff team members professional practice, including what went well, or not so well, and where improvements could be made i.e where unacceptable working practises are identified, implementing, and agreeing staff team member improvement plans, additional training, or mentoring.

To highlighting MAK CC commitment to on-going service improvement and quality delivery, care planning, person cantered support, mandatory training.

To monitor medications and give physical support to the staff team ensure stock checks are completed on a daily/weekly basis.

To ensure MARs sheets are completed and signed by staff team members at appropriate times throughout each shift.

Direct the shift, ensuring staff give support that follows individual's rota, care plans and daily support plans, with-in an identified area of the service.

Provide effective coaching to staff team to ensure pro-active and appropriate support to individuals within the service.

Ensure all communal/ staff areas remain clean, tidy, and welcoming to PWS and guests.

Provide on call cover as arranged by the management team, working within on call guidance, on a rota basis, covering evenings and weekends as appropriate to meet the needs of the service.

Ensure good team working, motivating, support and mentor to the staff when needed.

At each shift ensure staff team members complete all necessary documentation throughout the shift to an acceptable standard.

Support



Support people with regards to decision making and lifestyle choices enabling them to take control of their own lives, increasing independence.

Ensure all personal care and medication tasks are carried out in line with the needs, wishes and preferences of the individual, following the care/support plan.

Support people to take an active role in the community by accessing local facilities and wider community resources in the spirit of social inclusion.

Ensure the security of the building and the safety of the people living within their own homes.

Provide training and support to the staff team to develop, maintain and review person-centred plans & positive behaviour plans (PBS) ensuring everyone's plan has meaningful and achievable goals.

Support people to develop and expand social networks and friendships.

To support people through the transition process and to settle into their own homes and with homes with others.

Work to coordinate and develop the support to the individual including Health Action Plans and Person-Centred care/support plans and positive behaviour plans (PBS).

Contribute to / attend reviews and multi-disciplinary meetings.

Work with individuals to promote healthy lifestyle by liaising with local health care professionals.

Work in partnership with external advocates, social workers, CPN's, ICB, registered manager, service manager/s and other professionals to maximise quality of life for individuals.

Develop positive relationships with family and carers.

Work in line with the company's Adult Protection and Prevention of Abuse Policy and Whistle Blowing Policy to ensure that people are always kept safe.

To ensure all areas of work adhere to Health & Safety at work guidance and if any concerns to report these to a manager.

To maintain and follow Fire Safety guidance and ensure all fire safety equipment is regularly tested and in good working order (These tests must be recorded in Fire Safety documents on a monthly basis.

At all times to work within equal opportunities policy and procedure.

Ensure risk assessments are identified, produced, and actioned as necessary, in line with policies and procedures.

To be always flexible and responsive to meet the changing needs of the service and the people we support,

including evening and weekends to meet the needs of the service.



Ensure staff team members are administering and documenting all medication in line with the MAK Medication Administration policy and the individual's care/support plans.

Adhere to the Skills for Care Codes of Practice and MAK employee/staff handbook.

At all times work within current legislation.

Enter actively into supervision and appraisal.

Attend training on a regular basis to ensure that all mandatory and personal training and development needs are met in line with company requirements.

Finance

Support people to manage their own resources and finances.

To give practical support to individuals in line with their care/support plan and policy.

Ensure Petty Cash monies are checked, balanced, and recorded at the end of your shift and all financial discrepancies are reported to the manager/s immediately.

Administration

You will be required to work flexibly on a rota basis to meet the needs of the service, this includes weekends/evenings and bank holidays.

Ensure work is recorded accurately and appropriately in compliance with the company requirements and that records made, and personal information used are following the General Data Protection Regulation and the standards of information governance (GDPR).

Qualifications



Strong communication skills to effectively support team members and clients Proven leadership experience in a mental health or care setting Familiarity with developing and implementing care plans Proficient in IT systems relevant to mental health support Ability to manage a team in a high-pressure environment
