Job Description


We are looking for an individual who will support the needs of residents of of one of our properties within Concept Housing CIC ```Duties:``` To assist and enable residents to - maintain their accommodation - set and achieve goals and expectations on a short, medium and long term basis - facilitate their introduction to work and independent living - learn about budgeting, shopping, cleaning, health and well being - to find effective solutions when dealing with anti-social behaviour - resolve rent/service charge arrears - track their progress on a weekly, monthly and quarterly basis - promote independence and well \xe2\x80\x93 being - advocate in matters relating to the resident such as welfare benefit advice, behaviour intervention, arrears advice etc and to actively involve Management when issues cannot be resolved
- Provide support and assistance to residents with physical or intellectual disabilities
- Help individuals develop and maintain independent living skills
- Accompany individuals to appointments when required
- Document and report any changes in behaviour or health status - maintain weekly support sessions with residents - monitor and assist in progress of goals - maintain and update all records of residents - risk assess and promote security and safety with residents - identify and maintain focus on progression and achievement of personal goals for residents - liaise and develop relationships and work collaboratively with statutory and non-statutory / voluntary agencies where appropriate to negotiate and agree their participation in working with the residents ```Qualifications:```
- minimum 2 GCSE or equivalent
- Previous experience working as a support worker with individuals is essential
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
