Job Summary

The role involves supporting young people aged 16+ as they transition into independent adulthood. You'll act as their main point of contact, offering practical, emotional, financial, and social guidance. Key responsibilities include creating and reviewing tailored support plans, helping them engage in education or work, managing finances, and building essential life skills. You'll work closely with social workers and partner agencies, maintain accurate records, and advocate on behalf of the young person. The role also requires flexibility to work within a 24-hour rota and a commitment to safeguarding, inclusion, and continuous professional development.

Duties

Support young people aged 16+ with their transition to independent living. Provide emotional, practical, and financial guidance tailored to individual needs. Create and review personal development plans, promoting education, work, and life skills. Work with professionals and advocate on behalf of young people. Maintain accurate records and follow safeguarding, GDPR, and rota-based duties.
