We are seeking a compassionate and dedicated Support Worker to provide high-quality care and assistance to an individual within their own home. Duties
Support individuals with additional learning needs to live a fulfilling life
Prompt personal care and support needs, and provide hands-on support when required.
Administer medications following care plans and HIPAA guidelines to maintain safety and compliance
Support with the preparation of nutritious meals aligned with dietary requirements, ensuring proper food handling and sanitation practices
Conduct observations to identify changes in condition or behaviour, especially for those with dementia or autism
Support during community outings or social activities to promote engagement and socialisation
Maintain detailed documentation of care provided, incident reports, and updates in resident care plans
Requirements
Prior experience working in assisted living facilities, nursing homes, group homes or home & community care environments preferred
Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information
Strong communication skills to effectively collaborate with team members, service users, families, and healthcare professionals
Experience working with people with developmental disabilities or autism is highly desirable
Valid driver's license may be required for community outings or home visits
