We are seeking compassionate and dedicated Support Workers to provide high-quality care and assistance to individuals in various settings, including residential facilities, community homes, and home care environments. Duties
Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
Provide patient observation and monitor health status, including vital signs and overall well-being
Administer medications following care plans and HIPAA regulations
Support clients with meal preparation, cooking, and nutritional needs
Assist with mobility using equipment such as Hoyer lifts or resident lifts; heavy lifting may be required
Maintain a clean and safe environment through light cleaning and sanitation tasks
Implement individualized care plans tailored to each resident's needs, including dementia, Alzheimer's, autism, or developmental disabilities care
Support clients during activities that promote social engagement and community participation
Observe and report changes in behavior or health status to healthcare professionals or supervisors
Adhere to all safety protocols including CPR and first aid procedures; ensure compliance with HIPAA standards
Experience
Prior experience working in assisted living, nursing homes, hospice care, or home & community care is preferred
Knowledge of dementia care, memory care, Alzheimer's care, or working with people with developmental disabilities is advantageous
Experience with patient monitoring, medication administration, and creating or following care plans is desirable
Familiarity with HIPAA regulations and patient privacy standards is essential
Previous roles involving patient care, caregiving, or social work demonstrate a strong foundation for this position
Certification in CPR and First Aid is highly recommended; ongoing training opportunities are available for professional development
