We are seeking a dedicated and compassionate Support Worker to join our team. The ideal candidate will play a crucial role in assisting individuals in navigating their housing needs and overcoming personal challenges. This position requires a strong understanding of social work principles, crisis management, and housing regulations to effectively support clients in achieving stability and independence. Also provide them with support with their recovery from addiction journey and / or poor mental health.
The role will be for 10 hours per week worked flexibly with possibility to increase this to 15 hours per week at times depending on cover needed for annual leave in the team.
Duties
Provide direct support to clients in various settings, helping them access resources and services.
Engage in crisis intervention and management, utilizing motivational interviewing techniques to empower clients.
Conduct assessments to identify client needs and develop personalized support plans.
You must be non-judgmental, empathetic, caring and committed to enabling people to living their best life.
Also you will support and empower our service users to take back control of their lives while regularly assessing risk management plans for our high-risk residents.
You must have a high level of mental health experience and a good knowledge of controlled substances to assess risks.
You should be an efficient communicator and have good organisational skills to liaise with external agencies and internal stakeholders
You must be able to conduct one to one meeting with residents to give them the chance to offload
You must be confident in managing challenging behaviours
You must have housing and health and safety knowledge around shared accommodation.
You must hold a driving license to support service users and inspect our homes
Good IT skills for reporting including Teams, Sharepoint, Microsoft Office suite

About us
We are Telford After Care Team, a charity based in Telford that offers support to adults with addiction recovery and mental wellbeing since 2012. We offer one-to-one support, peer led group work, volunteering options and training. Most of our staff members have a lived experience that gives them the best understanding of the difficulties service users face. We also offer housing to people struggling with homelessness. Our programme is 2 years long and it is offered to over 18 years old. You will have 5 homes with 3-4 residents in each to support alongside the team
