Job Description

Job Purpose



To work collaboratively with the staff team and volunteers to provide high-quality day care support to service users, including the planning and delivery of meaningful activities, creation and maintenance of digital care plans, and assistance with transport and daily routines. The role involves emotional and practical support, supervision of volunteers, and working flexibly to ensure the Centre operates smoothly, safely, and in line with best practices.

Major Tasks



Key Responsibilities:



Care Planning & Service User Support

Create, update, and maintain accurate, person-centred individual care plans using digital tools. Maintain all documentation, including referral forms and records, to professional standards using computer systems. Offer practical and emotional support to service users with diverse and often complex needs. Communicate effectively with the Day Care Manager and staff about any changes or concerns.

Activities Programme



Develop and deliver a structured, responsive daily programme of activities in collaboration with the Activities Coordinator and volunteers. Undertake quarterly consultations with service users to inform programme development. Ensure activities are inclusive, appropriate, and meet individual needs.

Volunteer Coordination



Conduct daily briefings and provide regular supervision to a team of volunteers. Allocate tasks based on skills and aptitudes, ensuring volunteers feel valued and supported. Assist in training and skills development for volunteers.

Passenger Assistant Duties

(In the Absence of the Passenger Assistant)

When required, the Support Worker may be expected to undertake transport support duties to ensure the safe travel of service users to and from the Day Centre:

Assist service users during transport, ensuring their safety, comfort, and dignity at all times. Help individuals board and disembark vehicles safely, including appropriate use and handling of mobility aids. Ensure passengers are securely seated, and any equipment is safely stored or secured before the vehicle moves. Provide reassurance and emotional support to service users during journeys. Communicate effectively with drivers, carers, and Day Centre staff regarding any concerns, incidents, or observations during transport. Stay attentive to each service user's specific needs, including mobility challenges, medical conditions, or behavioural considerations.

Operational & General Duties



Manage your workload independently and proactively within a busy care environment. Use IT confidently for all aspects of planning, communication, and record-keeping. Support safe transport and assist during meal services as required. Adhere strictly to safeguarding, confidentiality, and health and safety standards. Lock up and set alarms as part of rota duties. Work weekends and flexibly as per the staff schedule.

Manage and prioritise workloads to ensure the smooth running of the service

Work flexibly and cooperatively with service users, volunteers and staff Maintain positive and constructive relationships with service users, volunteers and staff Ensure that all volunteers are up to date with all relevant information Undertake professional development and training as agreed with the Day Care Manager Maintain confidentiality, as appropriate, with sensitive or privileged information Adhere to all internal St. David's policies, procedures, systems and guidelines and contribute constructively, as required to their ongoing development Comply with all relevant laws, regulations, and legal responsibilities associated with the role. If necessary, from time to time, work outside normal hours for which time off in lieu (TOIL) will be given Secure premises, including setting the alarm Have regard for personal health and safety and that of those around you

Person Specification - Essential Requirements:

SVQ3 in Social Care (or equivalent recognised qualification in health or social care) High level of computer literacy - ability to use digital care systems, email, and databases confidently on a daily basis Proven ability to create and manage care plans in a professional care setting Experience supporting older or vulnerable adults in a structured care environment Ability to relate well to service users and volunteers
Ability to work independently, manage competing priorities, and remain calm under pressure

Strong interpersonal and communication skills Flexibility to work weekends and adapt to changing rota patterns

Desirable:



Experience supervising or working alongside volunteers Familiarity with activity planning and group facilitation Experience working in a day centre or similar setting Experience of working with volunteers would be advantageous A willingness to be adaptable and flexible is essential, as is a caring attitude
Job Types: Full-time, Fixed term contract
Contract length: 12 months

Pay: 25,885.00-29,248.00 per year
