Support Workers - Supported Living & Care Leavers
Overview
We are seeking compassionate and dedicated Support Workers to provide high-quality care and support within our supported living services. This includes working with individuals across a range of needs, including young people aged 16-18 who are care leavers transitioning to independence. The role focuses on promoting autonomy, safeguarding wellbeing, and delivering person-centred support in a safe and nurturing environment. Experience in supported living, care leaver services, learning disabilities, autism, or mental health is highly desirable.
Key Responsibilities
Provide emotional support and practical guidance to young people aged 16-18, helping them build life skills and confidence
Promote independence through daily living tasks, tenancy support, and community engagement
Assist with medication administration in line with care plans and protocols
Help with meal preparation and household tasks tailored to individual preferences
Support mobility needs and ensure safe transfers, including use of equipment
Respond to behaviours with empathy and effective strategies, especially in clients with autism, trauma backgrounds, or mental health needs
Accompany clients to appointments, education, training, or leisure activities
Maintain accurate records and report concerns to senior staff promptly
Apply first aid when necessary and uphold health and safety standards at all times
What We're Looking For
Experience in supported living, care leaver services, or similar care environments
Understanding of person-centred care, safeguarding, and support planning
Skills in behaviour support, medication administration, and first aid (training can be provided)
Ability to assist with mobility and undertake manual handling tasks safely
Full UK driving licence (desirable but not essential)
Excellent communication skills and a compassionate, proactive attitude
Ability to work independently and collaboratively within a dynamic team
Flexibility to work shifts, including evenings and weekends
