Job Title: Support Worker (Outreach)



Reports to: Manager

Hours: 20 - Monday to Friday

Job Purpose



The role of the Outreach support worker is to provide a mixture of coordinated supports to people, their families or carers supporting them to remain within their own homes and improve outcomes.

To work in partnerships with the person, their network and other agencies to strengthen individual capacity, enhance relationships/networks and increase engagement in their local communities to decrease social isolation.

Driving License Required.



Principal Accountabilities



Supporting Individuals



Engage with the individual, developing and maintaining relationships built on mutual respect, trust and the strengths of the individual. To encourage independence and self-reliance, helping the individual to develop positive strategies aimed at finding solutions to overcoming challenges. To promote and facilitate understanding the needs of the individual. Use a strengths based and solutions focused approach to enable people to make positive changes in their lives. To deliver support packages tailored to the individual. Enabling involvement and participation. Make referrals to specialist and other services for individuals where appropriate. To work in partnership with a variety of agencies including social work, health and voluntary sector to support individuals in their local communities.

Person Centered Assessment



To undertake person centered assessments at the beginning of involvement. To engage with individuals to identify goals, assess options and review outcomes. Participate in multi-agency meetings pertaining to individuals where required.

Other Duties and Responsibilities



To participate actively and constructively in meetings when required. To agree with the individual, their families or carers how personal information will be used, recorded and shared with others within the bounds of confidentiality and professional boundaries when working with families. To maintain accurate written and electronic records of work undertaken. To participate in individual supervision and team meetings. To maintain up to date knowledge and skills as required by the role. To maintain registration status with professional bodies as required by the role. To alert the Manager of any significant changes or events which affect the implementation of an individuals agreed plan or impact upon their health or wellbeing. To undertake other duties and responsibilities appropriate to the overall purpose and principles of the role.
Job Type: Part-time

Pay: 12.60 per hour
