We are seeking compassionate and dedicated Support Workers to provide high-quality care and assistance to individuals in various settings, including residential homes, community environments, and healthcare facilities. This role is vital in supporting clients with diverse needs, promoting independence, and enhancing their quality of life. The ideal candidate will have a strong commitment to patient-centered care, excellent communication skills, and the ability to work effectively both independently and as part of a team. Prior experience in home health, assisted living, or long-term care is highly valued.
Responsibilities
Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
Monitor and observe patients' conditions, including vital signs and overall well-being
Administer medications and document care activities in compliance with HIPAA regulations
Support patients with memory care needs, including dementia and Alzheimer's care
Prepare nutritious meals tailored to individual dietary requirements and assist with meal consumption when necessary
Utilize equipment such as Hoyer lifts or resident lifts to safely transfer clients with mobility challenges
Maintain a clean and safe environment through regular cleaning and sanitation practices
Follow individualized care plans developed by healthcare professionals or social workers
Provide emotional support and companionship to enhance mental well-being
Document patient observations accurately and communicate effectively with healthcare teams
Skills
Certification in First Aid and CPR required; additional certifications such as HIPAA training are preferred
Experience working in group homes, assisted living facilities, nursing homes, or home & community care environments
Knowledge of patient care procedures including medication administration, vital signs monitoring, and dementia or Alzheimer's care
Ability to assist with complex tasks such as heavy lifting or using specialized equipment like Hoyer lifts
Strong observational skills for patient monitoring and recognizing changes in condition
Compassionate attitude with excellent interpersonal skills for working with people with developmental disabilities or autism spectrum disorders
Ability to work independently while adhering to care plans and safety protocols
Prior experience in hospice care, long-term care, or social work is advantageous
Familiarity with home health practices, assisted living standards, and patient privacy regulations such as HIPAA
