We are seeking compassionate and dedicated Support Workers to provide high-quality care and assistance to individuals in various care settings. The ideal candidate will be committed to promoting independence, safety, and well-being for residents with diverse needs, including seniors, individuals with disabilities, and those requiring memory or dementia care. This role involves working closely with clients to support their daily activities and ensure a safe, comfortable environment.
Responsibilities
Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting
Provide patient observation and monitor health status, including vital signs and medication administration
Support residents with mobility using equipment such as Hoyer lifts and assistive devices
Prepare nutritious meals in accordance with care plans and dietary restrictions
Maintain a clean and safe environment through regular cleaning and sanitation tasks
Follow care plans meticulously to deliver personalized support tailored to each individual's needs
Document patient progress and observations in compliance with HIPAA regulations
Assist with transportation or outings within the community as needed
Respond promptly to emergencies, providing first aid or CPR when necessary
Collaborate with healthcare professionals, social workers, and family members to ensure comprehensive care
Experience
Candidates should have relevant experience in home & community care, assisted living, nursing homes, or long-term care environments. Prior experience working with people with developmental disabilities, autism, Alzheimer's or dementia is highly valued. Knowledge of medication administration, vital signs monitoring, patient observation, and care planning is essential. Experience with hospice care, home health services, or social work is a plus. Certification in CPR and First Aid is preferred. Candidates must demonstrate patience, compassion, excellent communication skills, and the ability to work effectively both independently and as part of a team. Heavy lifting capability may be required for certain tasks involving resident transfers or mobility assistance. Prior familiarity with HIPAA regulations ensures confidentiality and privacy of patient information.

Pay: 12.21-38.16 per hour
Expected hours: 6 - 60 per week
