We are seeking compassionate and dedicated Support Workers to provide high-quality care and assistance to individuals in various care settings. This role involves supporting residents with daily activities, health monitoring, and emotional well-being, ensuring a safe and supportive environment. The ideal candidate will have experience in home and community care, assisted living, or long-term care environments, with a strong focus on patient-centered support.
Duties
Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility.
Provide patient care including vital signs monitoring, medication administration, and health assessments.
Support residents with memory care needs, dementia, Alzheimer's, and autism spectrum disorders.
Prepare nutritious meals and assist with meal planning according to individual care plans.
Maintain a clean and safe environment through light cleaning and sanitation tasks.
Observe and document patient behavior, health status, and progress in accordance with HIPAA regulations.
Assist with the use of mobility aids such as Hoyer lifts and perform heavy lifting when necessary.
Support residents during outings or community activities to promote social engagement.
Follow all safety protocols including CPR and First Aid procedures to ensure resident safety at all times.
Collaborate with healthcare professionals to develop and implement personalized care plans.
Ensure compliance with all relevant regulations including HIPAA standards for confidentiality.
Skills
Proven experience in home health, assisted living, nursing homes, or hospice care.
Knowledge of patient observation techniques and monitoring vital signs.
Ability to assist with ADLs and provide compassionate patient support.
Strong understanding of dementia, Alzheimer's, memory care, and autism spectrum disorders.
Certifications such as CPR and First Aid are highly desirable.
Experience working with people with disabilities or developmental disabilities preferred.
Excellent communication skills to interact effectively with residents, families, and healthcare teams.
Ability to perform heavy lifting safely using equipment like Hoyer lifts when required.
Knowledge of HIPAA regulations to maintain confidentiality of patient information.
