Salary:

26,403 to 28,598 (pro rata) + enhancements for weekend and out of hours working

About the Role





We're looking for compassionate and dedicated Support Workers to help adults live independently and safely in their own homes, including people leaving hospital and people with complex and changing needs. You'll provide personal and practical support tailored to individual needs, promoting choice, dignity, and wellbeing working to an agreed Reablement plan.


Key Responsibilities




Deliver personal care and practical support in the community. Encourage individuals to achieve goals outlined in their support plans. Promote healthy living, including dietary support. Respond to urgent care needs and safeguarding concerns. Work flexibly and collaboratively with families and professionals. Flexible working including regular evenings (until 10pm) and weekends as part of a 7 day rota

What We're Looking For




Essential Experience & Skills:




Experience in care or support work (paid, voluntary, or informal). Calm under pressure and able to work independently and in a team. Comfortable with personal care and behavioural challenges. Strong communication skills (verbal and written). Understanding of confidentiality, dignity, and equality.

Additional Requirements:




Full UK driving licence. Willingness to undertake relevant training (e.g. Diploma in Health & Social Care). Physically able to assist with moving and handling tasks.
