The Health Care Support Worker plays a key role in empowering individuals to live as independently as possible and enhance their overall quality of life. The role focuses on providing practical assistance, emotional encouragement, and social support--not personal or clinical care. Working in partnership with individuals and multi-disciplinary teams, the Support Worker helps service users achieve their personal goals, maintain their independence, and engage with their communities. This includes supporting them with daily tasks, accessing services, and participating in meaningful activities.
Key Responsibilities
1. Support Planning and Enablement
Collaborate with individuals to develop person-centred support plans tailored to their needs, preferences, and aspirations.
Regularly review and adapt support plans to ensure they remain relevant and effective.
2. Emotional and Social Engagement
Provide reliable, friendly companionship to help improve mental well-being and reduce feelings of isolation.
Encourage and assist individuals to participate in social, recreational, or community-based activities.
3. Practical Daily Living Support
Assist with practical day-to-day tasks such as shopping, managing appointments, budgeting, and meal planning.
Support individuals in navigating public services, education, training, employment, and community resources.
4. Information, Advice and Advocacy
Empower individuals to make informed choices by supporting them in understanding their options and rights.
Liaise with external agencies or services on behalf of individuals to ensure they receive the appropriate support.
5. Communication and Record Keeping
Maintain accurate and timely records of support provided, goals achieved, and any changes in the individual's circumstances.
Share relevant information with colleagues and professionals to support coordinated care and continuity.
6. Training and Continuous Development
Take part in relevant training to build knowledge and keep up-to-date with best practice in the support sector.
Reflect on practice and take part in supervision and development reviews.
7. Health, Safety and Risk Awareness
Promote a safe and supportive environment for individuals and staff by adhering to health and safety procedures.
Identify and report risks, concerns, or incidents promptly in line with organisational policies.
8. Collaborative Working
Work closely with nurses and other professionals to deliver a joined-up, person-centred support experience.
Attend meetings, reviews, and briefings to contribute to service improvements and client progress.
Essential Requirements
Strong interpersonal skills with a respectful and non-judgmental approach
Ability to build rapport and develop trusting relationships
Awareness of the challenges faced by those requiring support and a genuine desire to help
Ability to work on own initiative and as part of a team
Good organisational and time-management skills
Basic IT proficiency for reporting and documentation
Prior experience in a support-based role
Understanding of local community services and networks
A relevant qualification (e.g. Level 2 in Health and Social Care) or willingness to work towards one
Working Conditions
May include working evenings, weekends, and flexible hours based on individual needs
Travel may be required within the local community to support clients with appointments and community engagement
