We are looking for a compassionate and proactive Support Worker to assist a young woman with several complex conditions including Cauda Equina, paraplegia at T5, dissociative disorder and functional seizures.

She lives on a small holding with her husband, two young children, 2 dogs, 5 cats and some chickens. This is an exciting opportunity to join a new care team to support this woman in all aspects of her daily living and accessing the community.

Location:

Whitemoor, St Austell

Roles Available:



Full time - 36 hours / 3 shifts per week Days only

Shift Patterns:



Day Shifts: 09:00-21:00

Pay Rates:



15.00 per hour weekday day rate 16.00 per hour weekend day rate
1.00 per hour less during probationary period

Role Responsibilities:



Providing clinical support with multiple health conditions Supporting with daily living and household duties Assisting with community outings and days out

Ideal Candidate



Essential:



A positive and proactive approach Previous care experience Flexible and reliable Comfortable around animals

Preferred:



A full, clean driving licence and willingness to drive an adapted vehicle Previous experience of complex care

JOB DESCRIPTION:



Job Purpose:

To support your client with their day-to-day living, promoting their independence and supporting them to live a fulfilled life ensuring their safety and wellbeing at all times.

Internal and Management:

This role will report to: Registered Manager/Care Manager

Key Duties/Responsibilities for our Support Workers:



Supporting your client to safely lead a fulfilling and more independent life by:

Provide practical support for your client and their family, such as helping with personal care, cooking, household tasks and paperwork Assist and encourage your client to engage in everyday home, social, leisure and work activities, according to the individual's choice Support and monitor your client's healthcare needs, including administering medication and other clinical tasks Help your clients to access community facilities and be included in community groups, if they so wish Understand their communication needs and adapt your own communication style accordingly Keep accurate, up to date records of all the care you provide Work positively with other professionals, such as doctors and therapists, to ensure consistency of support Maintain confidentiality and discretion at all times Participate in learning and development opportunities as required

As a term of employment, you may be required to undertake other duties and/or hours of work as may be reasonably required. This forms part of your employment contract.
