Purpose of the role
To support and work with care leavers and/or residents with low level learning disabilities to maintain their accommodation and links within the community. Supporting them to gain life skills, training, volunteering and/ or employment to progress into independent living.
Summary of Main Duties
Assess the practical and emotional needs through the progression plan for each client. Create an action plan including short term and long term goals with the client and support/ guide them to complete each action.
Complete and review client risk assessments, take appropriate actions when/if the risk level changes. Where necessary, work in partnership with other professionals and support agencies on behalf of the client.
Work as part of a hard working team and help seek opportunities in the community for clients and support them to engage and participate.
Support residents to manage and maintain all areas of their tenancy with Populus Housing.
Key areas of Responsibility
To ensure that the agreed support for clients is available, providing information and advice on the identified actions.
To build a positive relationship with clients and act as the first point of contact for client caseload.
To provide support and give appropriate advice to complete actions identified on clients progression plans. Support in areas including; budgeting and money management, employability, self care and life skills, motivation, emotional and mental health, physical health and managing tenancy and accommodation.
To support clients to maintain the whole property and support the work of the life skills mentor.
To encourage and support clients to attend appointments including job centre, doctors and mental health.
To maintain and update client records including; progression plan, action plan, visit logs, risk assessment and other evidence.
To support client transition into the service and out of the service when tenancy ready, provide floating support for up to 6 weeks after move on.
To establish and maintain good working relationships with statutory, private and voluntary organisations, particularly police, housing, social care, health, education/training, and employment organisations.
To communicate and engage with local communities to develop their understanding of the needs of our clients.
To operate within financial controls and budgetary systems to ensure the proper accounting of service resources.
To adhere to the policies and procedures as specified by the Company.
To undertake other duties that may be required by your line manager which are commensurate with the position.
Undertake regular personal supervision and performance management whilst attending team meetings.
Undertake training as identified by your line manager.
Skills & Knowledge
A proven ability of working with clients. Experience and understanding of issues facing vulnerable adults and their families.
An interest in people and a commitment to helping others.
The ability to communicate clearly and sensitively with vulnerable people and their families and other organisations.
Awareness and understanding of Mental Health.
An understanding of the benefits system and tenancy management.
Experience
At least 2 year's previous experience working in a position particularly with those who suffer from discrimination and require support relating to housing and homelessness and at last of one of the following: Young People, Housing, Mental Health and/or Learning Difficulties.
A relevant qualification in community work, social work/care, or a related field.
A full UK driving licence with business insurance and access to a vehicle is required.
