Position Summary:

We are seeking compassionate and dedicated Disability Support Workers to join our team. The role involves providing high-quality care and support to children, young adults and adults with disabilities, fostering their independence, and enabling them to participate fully in their communities.

Key Responsibilities:

Provide day-to-day support which may include personal care, meal preparation, and household tasks and community tasks.

Assist customers in accessing community services, activities, and social engagements.

Develop and maintain positive relationships with clients, families, and care teams.

Implement personalized care plans to support clients' goals and independence.

Monitor and report changes in clients' health, behaviour, or circumstances.

Maintain accurate and up-to-date client records in compliance with organizational policies.

Promote a safe and inclusive environment for clients and team members.

Desirable Qualifications and Experience:

Relevant qualification in Disability Support, Community Services, or equivalent (or willingness to obtain).

Previous experience in disability care or a similar role (preferred but not essential).

Full training will be provided.

Valid UK Driver's License and access to a reliable vehicle.

National Police Check and Working with adults and Children Check (or willingness to obtain).
